The formulary contains more than just covered medications. It details the cost-sharing structure for each drug, including deductibles, copays, and coinsurance amounts you’ll pay. Additionally, formularies outline important coverage rules that affect how you access your medications.

Three key rules commonly found in Part D formularies are step therapy, prior authorization, and quantity limits. Step therapy requires you to try the most cost-effective medication first before moving to more expensive or higher-risk alternatives if the initial treatment proves ineffective. Prior authorization means your doctor must obtain approval from the insurance company before prescribing certain medications.
